Agreement and Release of Liability
Reiki+Wisdom | Energy Healing & Counseling Services
This agreement is for Reiki and/or counseling sessions, not psychotherapy
Client understands that Rachel Pray is working only within the capacities of a Counselor and will not be held liable for discrepancies. While counseling can work with issues such as identifying and reaching life goals, and changing the behaviors that aren’t working well for you, counseling will not address psychological issues such as depression and anxiety. For issues such as these, seek the medical attention from a Physician or Licensed Mental Health Professional in your area. By signing this agreement, you are agreeing that you understand the difference in these two functions and you will get appropriate professional help for mental health issues if necessary. As a counseling client, your signature indicates that you have read this agreement for services carefully and understand its contents.
Privacy and Limits to Confidentiality:
Your privacy is of the utmost importance and all reasonable efforts will be taken to protect your personal information. Limits of confidentiality include storing of some personal information (name, address, Emergency Contact) in Counselor’s business files; risk of harm to self or others, as determined by Counselor, and any or all mental health crises that are not within the professional scope of Counselor to address. Client agrees to seek appropriate medical help if necessary to ensure emotional and physical safety. Counseling via computer, telephone and other technological means has limits to confidentiality and privacy that Counselor can neither determine nor control.
Release of Liability and Hold Harmless Agreement:
Client agrees to accept these terms and limits, and to hold Rachel Pray, MA and Reiki+Wisdom Energy Healing & Counseling harmless and release Counselor from liability in connection to any and all Counseling and Reiki Energy Healing services whether provided in person or through other any and all other means of communication, including emails, phone calls, texts, Skype, Zoom and other online services. Client understands and accepts that Reiki Energy Healing is not a substitute for healthcare or treatment of serious or life-threatening physical and mental health issues, trauma, substance abuse disorders, illness or injuries. Client agrees to seek the services of licensed medical professionals and/or licensed mental health professionals, as needed for treatment. Reiki+Wisdom Energy Healing & Counseling and Rachel Pray, MA reserve the right to refuse treatment to anyone and to terminate sessions with clients for any reason.
Payment and Cancellation Policy:
Client agrees to pay Rachel Pray/Reiki+Wisdom Energy Healing & Counseling in full before each and every individual session or treatment.
Counselor reserves the right to change weekly fee at any time in the future. Payment is to be made in full. No cash or checks will be accepted.
Client agrees to the following cancellation policy:
A cancellation fee of $25/session will be charged unless the client cancels less than 24 hrs ahead of the scheduled Reiki and/or Counseling session(s).
Cancellation can be made by email, text or phone. Client and Counselor can choose to end sessions at any time, without obligation or penalty.
